JDP 3 Form
Report of the Qualifying Examination and Advancement to Candidacy Committee For the Degree of Doctor of Philosophy in Evolutionary Biology.
Fill out the form and obtain your committee members’ and SDSU JDPEB Coordinator’s signatures on the day of your Qualifying Exam.  Obtain the UC Riverside JDPEB Advisor’s signature and then pay the fee at the UC Riverside Cashier’s Office. Return the form to Katherine Zumstein in LS 104 for processing.

Nomination of the Doctoral Dissertation Committee for the Doctor of Philosophy in Evolutionary Biology Degree.
Please complete the form and return it to Katherine Zumstein in LS 104 for processing.

NOTE: This form must only be completed if your committee membership has changed.

JDP 5 Form
Report on the final examination and filing of the dissertation for the Doctor of Philosophy degree in evolutionary biology.
Fill out the form and obtain your committee members’ and Dr. Elizabeth Waters’ signatures on the day of your Dissertation Defense. Obtain the UC Riverside JDPEB Advisor’s signature. Take the form to your final UC Riverside Office of Graduate Studies appointment.

JDP 6 Form
Annual Progress Report: report of research progress
Please complete the form, obtain the committee and advisors’ signatures, and e-mail it to Katherine Zumstein.

NOTE: Due to delays in the graduate offices, the appointment process often takes up to two months. Thus, the student must begin this process well in advance to ensure the committee is appropriately appointed before a qualifying exam or dissertation defense.
